In-Person Transaction at Parish Office: Online reservations and form submissions are currently not available. This page is provided for reviewing the official requirements, chapel guidelines, and offerings/rates. All memorial chapel reservations, occupancy coordination, scheduling of masses, and settlements are strictly conducted on a walk-in basis at the Immaculate Heart of Mary Parish Office during regular office hours.

Upon reservation of the chapel, the downpayment (1-day rate) must be paid. The remainder of the fees covering the entire duration shall be settled one (1) day before the interment.
The day of reservation is considered the first counting day of occupancy. Chapel use is counted per night of occupancy.
Chapel rental includes water dispenser, refill gallons of water, TV monitor, and up to two (2) celebrated Holy Masses.
Cancellation of reservation will incur a 20% deduction from the downpayment, which will be considered as a donation to the church.
The Parish Security Guard is the only person authorized to switch ON and OFF the air conditioning units.
Funeral services paraphernalia (outside stage lights, backdrops, etc.) are NOT ALLOWED inside the memorial chapels.
Cooking is strictly NOT ALLOWED inside the premises. Using sharp materials, nails, or adhesives to post on walls is prohibited.
Observe cleanliness and orderliness in the mortuary chapels at all times. Please dispose of all garbage properly in designated bins.
Any form of gambling, alcoholic drinks, and intoxicating beverages is strictly prohibited within the church grounds.
Observe proper parking and traffic rules within church premises. Please observe cleanliness and proper care in comfort rooms.
